Every time you visit campus you must complete this self-screening form. Please complete the form before arriving on campus and again just prior to leaving. Also, ensure you’ve completed the one-time COVID-19 education course before returning on campus.

Please Note: If you are a student worker coming on campus to work, please complete the employee self-screening form.

To help prevent the spread of COVID-19 and reduce the potential risk of exposure to our community, we are asking students participating in classes or programs on campus to complete a self-screening form. Your participation is important to help us take precautionary measures to protect you and everyone in the building.

1. Complete the COVID-19 education module

Students participating in classes or programs on campus must complete the COVID-19 education module in their class Canvas course prior to arriving on campus. Through the approximately 20 minute module, you will learn symptom detection, sources of high risk to COVID-19, and prevention measures. Please contact your instructor with questions.

2. Complete the self-screening form – Check-in section

Before arriving on campus, students will be required to fill out the student self-screening form to verify they are not currently experiencing any COVID-19 symptoms, nor have recently been around someone diagnosed with COVID-19.

3. Complete the self-screening form – Check-out section

Upon leaving campus, students will complete the process by filling out the student self-screening form again, this time to indicate they are checking out. The form will ask students to list buildings they visited and people they came into close contact with while performing their work.

Questions you will be asked when completing the student self-screening form

When you use the form to check in, you will be ask to:

  • Indicate whether you are checking in or checking out. Please select “Checking in” from the two options.
  • Give your name. Please include both your first and last name.
  • Give your phone number. Please include a number you can easily be contacted at.
  • Give your email address. It should be an email address you check often. It’s okay if it’s not your student email address.
  • Identify why you are coming on campus. Your options will include Class Lab, Testing, ESOL Orientation, or Other.
  • Review eight health-screening questions and verify you can answer “no” to all questions.
  • Indicate you have completed the required COVID-19 education.
  • Verify again you have answered “no” to the health screening questions. If you answered “yes” to any of the questions, or you have not yet taken the COVID-19 training, you must select that you understand you will need to stay home until you have completed the training and you can answer “no” to all screening questions.

When you use the form to check out, you will be ask to:

  • Indicate whether you are checking in or checking out. Please select “Checking out” from the two options.
  • Give your name. Please include both your first and last name.
  • Give your phone number. Please include a number you can easily be contacted at.
  • Give your email address. It should be an email address you check often. It’s okay if it’s not your student email address.
  • Select all building on campus you visited.
  • Enter what rooms you were in within each building, if known.
  • List any person you had close contact with while on campus.
