Online Forms

The following are commonly requested financial aid forms.  Note that some forms are year specific.  Please select the form for the appropriate academic year.

Deadline for Fall Quarter 2009 was June 1, 2009.
Required documents received after this date are late. Students need to be prepared to pay or set-up the Stepp payment plan Fall 2009 tuition (note: payment schedules for the 2009-2010 have not been determined at this time).

Verification Worksheets - in our efforts to reduce expenditures, the Financial Aid Office at Highline Community College is unable to make copies of tax returns and W2 forms. You must now provide the Office with copies we can keep.

Financial Aid Data Sheet

  • This form is used by the Financial Aid Office, along with the FAFSA results, to determine a student's intent to receive financial aid while attending Highline.

Verification Worksheets

If your Student Aid Report (SAR) or our office has indicated that you were selected for a process called "verification", please submit the completed worksheet below along with a copy of your tax return (and your parent's/spouse's if applicable).

Petitions

  • If you were informed that you are on Financial Aid Suspension and would like the opportunity to explain any unusual circumstances, please complete the Satisfactory Academic Progress Petition and return all documents to our office.

  • If you were informed that you have exhausted your financial aid eligibility at Highline Community College because you have "attempted" the number of credits beyond the required limit for your program, please complete the Petition for Extension of Financial Aid and return it to the Financial Aid office.

  • Loan Application (complete the Stafford Loan Workshop first)

  • If you are an Independent student and your 2009 anticipated income will be significantly less than your 2008 income, or if you are a Dependent student and your parents 2009 anticipated income will be significantly less than their 2008 income, you may complete the Change of Income form.  You may only  submit this form after you have received your 2009-2010 award letter.

  • Request of Change - Use this form if you are asking for  adjustments to your financial aid award or updating your mailing address and/or phone number.
